We require a $500(12 day) or $1,000(24 day) non-refundable deposit to hold your spot. We accept credit or debit cards over the phone, or a check with Student Application can be mailed to PO Box 1511, Pahoa, HI 96778.
Print, carefully fill out and sign these forms below with your son or daughter. Important details about our program are explained in our forms. We know it’s a lot to read, so thank you for reading them thoroughly! They’re critical to keeping our students safe. We’ve also included a packing list. The tuition balance and all forms should be received at least 30 days prior to course start.
SCUBA Forms- Only necessary for Wind and Water course. Medical Statement only needs a doctor’s signature if student answered “yes” to any of the questions.
